A little bit about my background & experience: I have been a qualified mental health nurse for the NHS for the last 10 years.
I first started working in an in-patient forensic unit that cared for people with enduring mental illnesses including psychosis, and brain injuries. I worked on both male and female wards, and individuals of all different ages. Patients suffered with a variety of mental health conditions, physical health problems, cognitive impairments and learning disabilities. I did this for 8 years.
For the last 2 years I have been a care co-ordinator, looking after individuals in the community, living with psychosis, or at risk of developing psychosis. I assess individuals referred to the team, to see if they meet criteria to come under the service. Individuals on my case load also had many other dual diagnosis, ranging from anxiety, depression, personality disorders, bipolar disorder, cognitive impairments, learning disabilities, drug & alcohol problems, and many other conditions/illnesses. Along side their mental health, I ensured their physical health was managed well, administering depots, medication overseeing/monitoring, and other life long conditions.
Some other support included graded exposure for individuals suffering from acrophobia, and anxiety disorders. I would also signpost and refer individuals to agencies and teams if I felt they needed psychological support, talking therapies, further assessment, drug and alcohol support, or group therapy. Whilst being a care coordinator my job was also to look after the individuals social aspects. Finding resources for them in the community gym memberships, finances, benefit support, housing and accommodation issues, finding courses and education, and helping individuals find employment.
I have worked collaboratively with individuals carers/family to share information, update and develop the best care for that individual. Ensuring they are always involved in developing the care that is the best suited for their loved one.
